Yet more major and very nit-picky refactoring for performance, etc. Also use std::atomic<> now with a TODO to implement a shim if we ever do need to build this on a pre-c++11 compiler.

timestamps changed from uint64_t to int64_t
...
There were cases in the code where time calculations and comparisons were overflowing and causing connection instability. This will keep time calculations within expected ranges.
